What is the sliding fee discount program?
The Sliding Fee Discount Program allows all patients (referred to as applicants) to receive healthcare services at a reduced cost based on income and family size. To be eligible for a Sliding Fee Discount, applicants must provide evidence of household income not exceeding double the published annual Federal Poverty Guidelines for the current year; which varies depending upon the size of the family and the number of people that live in the household.
Based upon income guidelines, the Sliding Fee Discount available may be a nominal charge. The nominal fee can be waived if there is financial hardship or if the fee is a barrier to accessing care. Additionally, applicants appearing to be at or below 100% of the Federal Poverty Level, will be referred to a representative who will assist the applicant in completing a health insurance application. Representatives will be available specific days at the Tri-County health centers in Cohocton, Wayland, Geneseo, and the Administrative Office in Dansville, as well as the Nunda Government Center site.
